Jaclyn M. Smith, LCSW-R is a licensed clinical social worker with over 20 years of experience providing psychotherapy and clinical supervision across community-based, supportive housing, and medical settings. Her clinical expertise includes working with individuals coping with chronic health conditions, aging, vision loss, and HIV, as well as those navigating the emotional challenges of long-term illness and life transitions.
Jaclyn began her career at the Center for Urban Community Services (CUCS), providing clinical services within supportive housing programs for individuals living with chronic mental illness. She went on to serve as Program Director at the Lighthouse Guild, where she oversaw mental health services for individuals who are blind or experiencing vision loss, helping clients adjust to changes in independence and identity. At Gay Men’s Health Crisis (GMHC), Jaclyn directed mental health and supportive services for clients living with HIV, integrating medical and psychosocial care to improve client well-being and stability.
In addition to these leadership roles, Jaclyn has maintained a psychotherapy practice focused on adults and older adults dealing with anxiety, depression, trauma, grief, chronic illness, and end-of-life concerns. She draws on evidence-based modalities, including CBT and mindfulness, to support clients in building resilience, managing stressors, and enhancing overall quality of life.
